Question: I suffer from osteoarthritis. My doctor says I should keep exercising even though my joints hurt. He says its okay for me to take a pain medication like Tylenol in order to remain active. But Ive read that Tylenol can damage my liver. What am I to do?

Answer: You are right to be concerned about medication safety. But your doctors also right that you need to be physically active. So how do you balance these seemingly competing objectives?

I posed your question to two specialists at Sunnybrook Health Sciences Centre: One is a surgeon who performs joint replacement operations and the other is an expert in drug safety.

Lets start with the surgeon. Dr. Jeffery Gollish, Medical Director of Sunnybrooks Holland Orthopaedic and Arthritic Centre, says he sees lots of people who feel the same way you do.

Many patients, he says, fear that exercise will lead to additional damage of their joints. The fact that it hurts when they move is taken as a sign that they are doing harm to themselves. And they worry medications that mask their pain will simply allow them to do even more harm.

The issues of exercise and medication are two areas where the public has misconceptions, says Dr. Gollish. They think both are harmful. In fact, both are beneficial.

Exercise, in general, is good and wont harm the joints. The only thing we tell people to stop doing is running and avoiding any impact activity.

Conditions Treated By Acetaminophen Vs Ibuprofen

Acetaminophen and ibuprofen are both effective pain relievers that are FDA-approved to treat mild to moderate pain and fever. Examples of mild to moderate pain include headaches, back pain, toothaches, muscle aches, sprains, and menstrual cramps.

Acetaminophen is only indicated for the temporary treatment of pain and fever. However, it also has off-label uses for arthritis, migraines, and dysmenorrhea . Acetaminophen may not be as effective as other drugs for these off-label uses.

Ibuprofen can be used to treat general acute pain and fever. It is also labeled to treat pain and inflammation from arthritis, migraines, and dysmenorrhea.

Research has also shown that acetaminophen and ibuprofen can be used to treat patent ductus arteriosus in preterm infants. The ductus arteriosus is a major blood vessel in the infant heart that normally closes after birth. However, in some babies, this blood vessel remains open and can cause heart complications. NSAIDs like ibuprofen or naproxen have been used to treat patent ductus arteriosus.

Dosage For People With Kidney Problems

Before prescribing Xarelto for you, your doctor will check to see how well your kidneys are working. This allows your doctor to know whether youll need renal dosing of Xarelto. Renal dosing is a reduced dose of the drug thats typically determined by calculating your creatinine clearance.

Creatinine is a waste product thats produced by your body. Your kidneys filter creatinine out of your blood, and it exits your body through your urine. Measuring the amount of creatinine in your urine allows doctors to see how well your kidneys are working.

Usually, your doctor wont prescribe Xarelto for you to treat or prevent a DVT or PE if your creatinine clearance is lower than 15 mL/min.

However, Xarelto can be used to reduce the risk of heart attack and stroke if you have kidney problems, no matter how low your creatinine clearance is. In this case, it can also be used to reduce the risk of heart attack, stroke, and other complications from blood clots.

If you have A-fib and your creatinine clearance is 50 mL/min or less, youll need a dose reduction of the drug. In this case, the recommended dosage of Xarelto is 15 mg taken once every evening.

Is Having One Or Two Drinks Ok

Its usually safe to have one or two drinks around the same time that you take an occasional dose of acetaminophen. Most people are fine taking acetaminophen and alcohol together as long as they take typical doses of acetaminophen and only combine them every once in a while.

If you are considering combining these substances, its best to do so only if youve been drinking a moderate amount of alcohol people assigned male at birth should have two drinks or less per day, and people assigned female at birth should have one drink or less per day.

If you get a nosebleed, pinch your nose just above your nostrils for about 10 minutes. Meanwhile, lean forward and breathe through your mouth.

However, if you have severe bleeding, you should call 911. Be sure to get emergency medical help for bleeding that:

  • seems very excessive
  • doesnt stop after 10 minutes of applying pressure
  • is caused by a serious injury or trauma to your head

What Happens With Blood Clots

    MONDAY, April 14, 2014 — People taking blood thinners to prevent clots in their legs or lungs may put themselves at risk for serious internal bleeding if they also take common painkillers such as aspirin or ibuprofen, researchers warn.

    These over-the-counter painkillers — called nonsteroidal anti-inflammatory drugs — include such drugs as Advil, Aleve, Bufferin, Excedrin, Motrin and Nuprin, as well as aspirin. Patients who use blood thinners such as warfarin, Eliquis, Xarelto or Pradaxa should not take them, the researchers said.

    “If you are taking an NSAID while taking an anticoagulant, your risk of a major bleed is double what it would be if you weren’t taking an NSAID,” said lead researcher Dr. Bruce Davidson. He’s with the division of pulmonary and critical care medicine at the University of Washington School of Medicine in Seattle.

    One-quarter of these major bleeds happen within eight days of taking an NSAID, Davidson said. “It’s not like you have to be taking them for weeks or months. Some bleeds happen with a single dose,” he said.

    The risk of bleeding might be even greater for people with an irregular heartbeat, called atrial fibrillation, who take a blood thinner and use an NSAID, Davidson said.

    “The message for people is if you’re taking a blood thinner, do not take an NSAID — just don’t take one,” Davidson said.

    “If you have a headache or aching muscles or joints, take generic Tylenol . Don’t take NSAIDs for casual use,” he added.
