TNF is a naturally occurring cytokine produced primarily by activated macrophages and T cells and exists predominantly as a trimer . Two distinct receptors for TNF exist naturally as monomeric molecules on cell surfaces and in soluble forms. One is a 55 kD protein and the other has a molecular weight of 75 kD . The biological activity of TNF is dependent upon binding to either cell surface TNFR. Monomers of the extracellular portion of the TNFRs naturally cleaved from the cell surface are termed soluble TNF receptors . sTNFRs bind with high affinity to circulating TNF and act as natural antagonists to TNF preventing the TNF molecules from binding to cell-bound receptors.

The dimeric structure of ETN enhances its binding affinity and provides substantially greater competitive inhibition of TNF than monomeric soluble receptors. Use of an IgG Fc region as a fusion element in this construction imparts a longer serum half-life compared with monomeric soluble receptors.

Etanercept has also been shown to be effective in psoriatic arthritis . In this12 week randomized, double blind controlled trial, 60 patients with PsA and psoriasis were studied for efficacy and safety of ETN or placebo. The PsA outcome measures included the proportion of patients who met the Psoriatic Arthritis Response Criteria and those who met the ACR20 response criteria. Psoriasis endpoints included improvement in the Psoriasis Area and Severity Index and improvement in prospectively identified individual target lesions. Eighty seven percent of ETN-treated patients met the PsARC compared with 23% of placebo-controlled patients. The ACR20 was achieved by 73% of ETN-treated patients compared with 13% of placebo-treated patients. Of the 19 patients in each treatment group who could be assessed for psoriasis , five of ETN-treated patients achieved a 75% improvement in the PASI, compared with none of the placebo-treated patients . The median PASI improvement was 46% in ETN-treated patients versus 9% in placebo treated patients similarly, median target lesion improvements were 50% and 0, respectively.

Enbrel For Polyarticular Juvenile Idiopathic Arthritis

Enbrel is also approved to treat moderate to severe polyarticular juvenile idiopathic arthritis in children ages 2 years and older.

Polyarticular JIA is a type of arthritis that occurs in people younger than 18 years old. It causes swelling and pain in five or more joints. Its not known what causes polyarticular JIA.

Although polyarticular JIA may cause pain in any joint, the most commonly affected areas are small joints in the fingers and hands.

Enbrel works by decreasing the swelling in joints. This helps make it easier and less painful for children with polyarticular JIA to move their affected joints.

Nutritional Supplements And Dietary Changes

There’s no strong evidence to suggest that specific dietary changes can improve rheumatoid arthritis, although some people with rheumatoid arthritis feel their symptoms get worse after they have eaten certain foods.

If you think this may be the case for you, it may be useful to try avoiding problematic foods for a few weeks to see if your symptoms improve.

But it’s important to ensure your overall diet is still healthy and balanced. A Mediterranean-style diet, which is based on vegetables, fruits, legumes, nuts, beans, cereals, grains, fish and unsaturated fats such as olive oil, is recommended.

I’m on the higher dose of Mrs and have now been put on enbrel after trying several other dmards with no success. I’ve only been on enbrel two weeks but I’m sure I can feel an improvement. Has anyone else had this happen or can anyone share their experiences with enbrel? Thanks.

    Hi Lauraine,

    I’m on Benepali which is a bio similar of Enbrel. I have been on it 5 weeks, but I noticed a difference almost immediately. It said 2-12 weeks to work, but it definitely started to work quicker.

    This is the first time all year I’ve not been on steroids and been pain free!

    Good luck with yours. I hope this continues for both of us!

    Hi and thank you for your response. I think the reason I was sort of shocked by this almost immediate reaction to enbrel was because previously I had tried Humira and this can take weeks to have any effect. It actually had no effect for me hence being put on the enbrel which I am glad to say does seem to be working for me!

    I’m glad it’s working for you too. And no side effects either. Bonus. Injection day tomorrow. Looking forward to it. No prednisolone either somy weight is steadily coming back down. Happy days. Thanks again for your reply.

What Else Should I Know

This medication should be refrigerated in its original container. Do not freeze this medicine. If you are traveling, you may store etanercept at room tempereature for up to 14 days. Once etanercept reaches room temperature, you cannot put it back in the refrigerator.

You must continue your regular visits to the rheumatologist. Your doctor will monitor you for any improvements in your rheumatoid arthritis and for any signs of infections.
