According to researchers, people with RA should be considered overweight with a BMI of 23 and obese with a BMI of 28, especially when it comes to assessing the risk of heart disease.

Combined Analysis Of Women With Ra And Matched Comparators

Figure 3 displays the multivariable HRs for the combined analysis of RA and matched comparators with stable weight comparators as the reference group. In this analysis, the 95%CIs were wide at the extreme weight change categories due to low sample size, particularly for women with RA. Women with RA had higher HR for mortality than comparators in every weight change category. The highest HR for mortality for both RA and comparators was in the severe weight loss category. Women with RA and severe weight loss in the peri-RA period had HR for mortality of 2.92 compared to comparators with stable weight. Comparators with severe weight loss had HR for mortality of 2.21 compared to comparators with stable weight. There was no statistical interaction for RA/comparator status and weight change category for mortality .

What Ra Does To Your Heart And Lungs

RA can affect more than just your joints. Complications can involve your organs, blood vessels, and bones.

RA can damage your lungs or inflame the lining around them. This causes chest pain that worsens with breathing, called pleurisy. Lung problems are the most common symptoms of RA outside the joints. This may not cause symptoms, or you might notice shortness of breath. Your doctor can treat it with drugs that ease the inflammation in your lungs.

Whatever type of diet you choose to manage your RA symptoms, just make sure its sustainable. Its about choosing something that you can actually adhere to that works with your lifestyle, Dr. Haberman explains.

If a certain diet feels too restrictive or overwhelming, focus on smaller swaps instead. Any healthy eating habits you adopt will do more to reduce inflammation and RA pain than making no changes at all.

One size doesnt fit all, and if you cant stick with a particular plan, thats okay, Dr. Haberman notes. Even if you simply make an effort to eat less added sugar or add another serving of vegetables to your day, its going to help you feel better, she says.

The Other Side Of The Coin: Rheumatoid Arthritis And Weight Gain

Clearly, unexplained weight loss with rheumatoid arthritis can be dangerous, but so is being overweight.

Fat tissue releases cytokines, which cause inflammation in the body, says Dr. Andrew. These cytokine levels are already elevated when someone has an inflammatory arthritis, so being overweight can exacerbate the already existing inflammation.

The physical toll of being overweight is also problematic. Carrying extra weight places increased pressure and stress on the joints, which can worsen the pain and stiffness associated with rheumatoid arthritis, says Dr. Andrew. Every pound of excess weight places about four pounds of extra pressure on the knees.

The result of all that physical pressure, adds Dr. Baker, is that many people with RA also end up with osteoarthritis . That combo adds up to higher rates of disability and higher pain scores and drug therapies can only help so much.

Studies have shown that some DMARDs may not be as effective in subjects who are overweight or obese. One study showed that regardless of the type of initial treatment, subjects with RA who were overweight or obese were significantly less likely to achieve sustained remission compared to subjects with a normal weight, says Dr. Andrew.

Meanwhile, youre more apt to experience drug side effects if youre overweight because obesity often causes inflammation in the liver that interferes with your ability to process drugs, says Dr. Baker.

The bathroom scale may not be the most reliable means of determining how much weight you may need to lose, say experts. Thats because body weight doesn’t take into account the proportion of fat in the body, or where that fat is deposited. Other measuring devices may deliver a more accurate assessment of body fat versus muscle mass, though they arent all easy to find.

Most experts agree that each method has its limitations, and sometimes the mirror test is the best one all.

BMI

Body mass index has largely replaced the number on the scale alone to determine overweight or obesity. Although it is relatively simple to calculate, it has its drawbacks. For one, it does not identify fat content, saysRobert Davidson, PhD, the program director of the Nutrition and Human Performance Program at Logan University in St. Louis.

A person could have a normal BMI but still need to lose weight, says exercise physiologist, Lauren McDonald, at Duke Health & Fitness Center in Durham, N.C. It’s one of the inaccuracies of BMI measurements. It depends on where the weight is being carried. Carrying excess weight in the abdomen is riskier to your health than excess weight around the hips and thighs. Bottom line: BMI is a good screening tool but not a diagnostic tool.”
