“Patients with new onset myasthenia who received rituximab as a complement to standard of care showed greater improvement compared with patients who were given a placebo,” says Fredrik Piehl, professor at the Department of Clinical Neuroscience, Karolinska Institutet, and the study’s principal investigator. “They also needed fewer adjuvant treatments and lower doses of cortisone than the placebo group. These are encouraging results that give hope for a more effective strategy for controlling new onset myasthenia more quickly, even if larger studies will be needed to assess the long-term effects of the treatment.”
After four months, 71 percent of the rituximab group had attained good control of their disease according to a well-established 13-item rating scale, compared with 29 percent of the placebo group. Later follow-ups at six, nine and twelve months produced similar results.

Some people with fatigue get better if they do an activity or a hobby something that makes them happy and something positive for them, encourages Dr. Rosian. If something makes you feel better and reduces your pain, its going to help you with your fatigue.

You wake up exhausted, drag yourself through the day, and keel over into bed with nothing left. This is rheumatoid arthritis fatigue. Many of us have been known to say that the pain is bad, but the fatigue is almost worse.
We are not talking about just being tired. RA fatigue is pervasive. To make others understand what it feels like, you can liken it to the fatigue that people feel when they have the flu. An editorial in the journal Rheumatologydefined fatigue as physical and/or mental exhaustion that leads to a marked impairment in normal activities, whether mental, physical, domestic, social, or occupational. It is not improved substantially by bed rest and may be worsened by normal levels of physical or mental activity.
Living with a chronic illness is exhausting, and chronic pain certainly contributes to feeling like a wrung- out dishcloth. However, there is also a biological reason for RA fatigue.
Understanding RA fatigue
At the 2015 annual Congress of the European League Against Rheumatism it was reported that not only is there a biological basis for the fatigue experienced by people with rheumatic conditions, such as RA, but that this fatigue is also gene regulated. A Norwegian researcher presented the results of a study, stating his belief that interleukin 1 beta is the fatigue cytokine.
What you can do about RA fatigue
But this isnt the case for everyone. In fact, levels of fatigue are not always associated with the level of inflammation.
Ineffective Strategies For Improving Fatigue In Ra
Tight control alone does not fully improve fatigue. At 12 months, 43% of RA patients experienced ongoing fatigue in a treat to target strategy study. Nearly one in four who were fatigued at the onset of the study showed lower levels of fatigue by 1 year, but one in six of the patients who were not initially fatigued recorded increased fatigue.
Treatment of anaemia should theoretically improve fatigue. RA can cause anaemia of chronic disease. A Cochrane review of erythropoietin for anaemia in RA did improve patient global assessment, but the results for fatigue were not provided or inconsistent.
An RCT of morphine and naloxone studied pain in various conditions including RA. Active treatment was comparable with placebo. An RCT of tetrahydrocannabinol for treating chronic pain in RA and ankylosing spondylitis is ongoing, and outcomes will include pain and fatigue. The design is innovative as cannabidiol nonresponders will have THC added to their treatment after 12 weeks.
