Disease-modifying antirheumatic drugs are used used to slow or stop rheumatoid arthritis by suppressing the immune system. The generic names for commonly used DMARDs include:

  • Hydroxychloroquine

Biologic drugs target and prevent a specific reaction from happening, stopping the inflammatory process.

This class of medications, called biologic response modifiers, is technically a subset of DMARDs. They may be used with traditional DMARDs or as an alternative to them. Biologics:

  • Disrupt certain parts of the cascade of events that lead to RA inflammation and have the potential to stop the disease process.
  • Increase a persons risk of infection and tend to be expensive. Because of these potential downsides, biologics are used when methotrexate or other DMARDs prove insufficient or cause unacceptable side-effects.
  • May become less effective and/or cause worsening side effects over time. The doctor and patient can work together to monitor changes and decide if and when switching medication is advisable.

Testing for TuberculosisBefore taking any type of biologic medication, a person must be tested for tuberculosis. People who have latent tuberculosis carry the Mycobacterium tuberculosis bacterium without having tuberculosis symptoms. If a person with latent tuberculosis takes immune-suppressing biologic medications the bacterium can multiply and cause symptomatic tuberculosis.

Topicals For Pain Relief

There are so many options for topical pain support available, and I feel like Ive tried a good variety of them:

  • Capsaicin cream
  • Icy Hot
  • Tiger Balm
  • BioFreeze
  • Arnica
  • Salonpas patches

Ive had some temporary pain relief and support from each of these, and I don’t think theres harm in trying a few different ones. My personal favorites have been Tiger Balm Active Muscle Gel and Arnica Cream . I usually keep a few different types in the house so if one isnt helpful when my pain is increasing, I have another option on hand. I like being able to target my uncomfortable joints topically rather than trying to take oral medication, wait for it to be absorbed, and then travel to the area in question.

Note: I always recommend checking with your doctor first before adding anything to your treatment regimen.

Aspirin: A Special Nsaid We No Longer Use For Pain

Aspirin is another analgesic available over-the-counter.

Its technically also a NSAID, but its chemical structure is a bit different from the other NSAIDs. This is what allows it to be effective in reducing strokes and heart attacks. It is also less likely to affect the kidneys than other NSAIDs are.

Aspirin is no longer used as an analgesic by the medical community. But many older adults still reach for aspirin to treat their aches and pains, because they are used to thinking of it as a painkiller. Aspirin is also included in certain over-the-counter medications, such as Excedrin.

Taking a very occasional aspirin for a headache or other pain is not terribly risky for most aging adults. But using aspirin more often increases the risk of internal bleeding. So, I discourage my older patients from using aspirin for pain.

In short, the safest oral OTC painkiller for older adults is usually acetaminophen, provided you dont exceed 3,000 mg per day.
