My doctor said Id probably dislocated my hip and sent me for X-rays. When the scans were done they found not only a dislocated hip but arthritis in four places on my spine and in my hip, he says. Shocked doesnt even begin to describe what I felt. Id always tried to live a healthy lifestyle and never had any real health problems before that, and now I have a chronic disease. Like, what?

Sam was referred to an orthopedic specialist who diagnosed him with ankylosing spondylitis and recommended surgery an idea hes not comfortable with yet. Its all very recent so Im still weighing my options for treatment, he says.

How Are Rheumatoid Arthritis And Psoriatic Arthritis Treated

Many of the same treatment options are used to treat both RA and PsA. Nonsteroidal anti-inflammatory drugs , such as aspirin and Ibuprofen, are useful for over-the-counter treatment of both conditions. Corticosteroid joint injections can provide quick relief for RA and PsA symptoms. Side effects associated with steroid use, and with oral steroids especially, mean these medications are not a good long-term option for treating joint pain. In addition, corticosteroid use can make psoriasis flare-ups more volatile in people who have PsA.

Methotrexate, a disease-modifying antirheumatic drug , is the most commonly used DMARD for RA. Methotrexate is sometimes prescribed off-label for PsA. However, it is not as effective for PsA as it is for RA.

Biological DMARDs, such as Enbrel , Remicade , Humira , Simponi , and Cimzia are approved by the U.S. Food and Drug Administration for moderate to severe RA and for PsA. Some of these drugs may be administered along with methotrexate. Unlike methotrexate, biological DMARDs have been shown to be effective in preventing bone erosion in people with RA and PsA, making these drugs especially helpful.

Inflammation in RA is associated with a different interleukin called IL-6. The IL-6 inhibitor drugs Actemra and Kevzara work against RA, but have not been proven to help with PsA.

In addition, Rituxan and Kineret can be helpful for RA that has not responded to other types of treatment. However, these drugs are not useful in treating PsA.

How Do You Differentiate Between Mechanical Arthritis And Inflammatory Arthritis

Whereas mechanical arthritis most commonly presents after the age of 50 and increases in frequency with age, inflammatory arthritis tends to involve a broader age spectrum, often striking patients in the peak working and child-rearing age group.

Mechanical arthritis is joint-localized, presents with spurs of new bone from the joints, and tends not be associated with systemic symptoms. However, patients with IA present with joint warmth , soft swelling of the joints, and constitutional symptoms such as fever, weight loss, malaise, morning stiffness greater than 30 minutes and generalized fatigue. These symptoms are accompanied by extra-articular manifestations such as nodules, eye inflammation, and skin abnormalities.

A simple test of the metacarpal-phalangeal joints can be performed to rule out osteoarthritis. In this test, the second through fourth metacarpo-phalangeal joints or the metatarso-phalangeal joints are squeezed together to test for tenderness. A positive result is an indication of the presence of inflammatory arthritis.

What Imaging Techniques May Be Used To Diagnose Arthritis

Imaging techniques may give your healthcare provider a clearer picture of what is happening to your joint. Imaging techniques may include the following:

  • X-ray. X-rays may show joint changes and bone damage found in some types of arthritis. Other imaging tests may also be done.

  • Ultrasound. Ultrasound uses sound waves to see the quality of synovial tissue, tendons, ligaments, and bones.

Caspar Criteria For Diagnosis

Diagnosing psoriatic arthritis relies on markers in an established system called the Classification Criteria for Psoriatic Arthritis .

The criteria are each assigned a point value. Each one has a value of 1 point except for current psoriasis, which has a value of 2 points.

The criteria are as follows:

  • current psoriasis outbreak
  • personal or family history of psoriasis
  • swollen fingers or toes, known as dactylitis
  • nail problems, like separation from the nail bed
  • bone growths near a joint that are visible on an X-ray
  • absence of rheumatoid factor

A person must have at least 3 points based on the CASPAR criteria to be diagnosed with psoriatic arthritis.

When Gideon M., 21, of Surrey, British Columbia, Canada, first started having hand and knee pain, his doctor wrote it off as a sports injury. Then one day he woke up and couldnt move his hands at all and his fingers looked misshapen. I was very adamant about making sure that I got appropriate testing done and even asked to get bloodwork and X-rays done once my fingers got crooked, he says. The diagnosis? Osteoarthritis. Rheumatoid Arthritis Classification Criteria

To help doctors make diagnoses, the American College of Rheumatology and the European League Against Rheumatism collaborated to create the 2010 Rheumatoid Arthritis Classification Criteria.

These criteria set a minimum standard for what signs and symptoms must be noted before RA can be diagnosed.2 A total point score of 6 or more indicates rheumatoid arthritis.

It Started With A Common Cold

Who knew a dumb cold could trigger arthritis? But thats exactly what happened to me, says Max P., 28. Her struggle with arthritis started with a slight fever, sore throat, runny nose, and a cough. It felt like every other cold shed had until it didnt let up, and her knees and ankles started to swell and hurt. Soon her hands and feet were so swollen she could barely function and her skin had taken on a scaly appearance. He told me at that point it didnt matter so much exactly what type of arthritis I had as much as accepting the fact I had it, she says. And that was the hardest part for me. Im not old, its so frustrating to feel like I am.

The doctor eventually found signs of arthritis in her spine as well and put her on methotrexate and prednisone, which has brought significant relief.
