Youre no stranger to the fact that there are multiple diet plans in the market. These include fad diets, low-carb diets, heart healthy diets, lemonade diets, juice diets, and the list goes on. Is there a specific diet plan for individuals living with psoriatic arthritis to follow? There are several, actually.

  • Paleo diet: Also known as the Caveman Diet, the Paleo diet avoids grains, nuts, processed foods, and dairy. Key staples of the Paleo diet include fish, eggs, meat, fruits and vegetables . The benefits of this type of diet for individuals living with psoriatic arthritis is that the main staples of the Paleo diet can reduce swelling often caused by processed and sugary foods and dairy.
  • Anti-inflammatory diet: One of the most prevalent symptoms of psoriatic arthritis is joint inflammation. Eating foods with anti-inflammatory properties can reduce swelling. Fatty fish, leafy greens, and a variety of fruits are great food choices for less inflammation.
  • Weight loss diet: To lose weight, you need to limit your intake of carbohydrates, sugars, and fats. Keeping your weight down keeps pressure off of your joints. Additionally, individuals who are overweight tend to have more severe forms of arthritis and are more prone to develop psoriatic arthritis.

Icipating Physicians And Patients

Rheumatologists and dermatologists were eligible to participate in the study if they had worked 3 years as a physician, had qualified between 1979 and 2012, and were responsible for treatment decisions and management of patients with PsA.

Patients were eligible for inclusion in the study if they were 18 years old with a physician-confirmed diagnosis of PsA and were not currently enrolled in a clinical trial. Patients were eligible for inclusion in the analysis if they were receiving TNFi for 3 months and completed the SF-36v2 health survey. There were no exclusion criteria in the study.

Patient characteristics were provided by physicians, covering demographics, comorbidities including fibromyalgia, disease status including duration of disease, presence of inflammation, presence of enthesitis and treatment details including type of immunomodulatory therapy and use of prescribed and non-prescribed pain medication. Patient-reported happiness was assessed using SF-36v2 question 9 How much of the time during the past week have you been happy? with possible responses: all of the time, most of the time, some of the time, a little of the time, none of the time.

Get Better Sleep With Psoriatic Arthritis

A recent study showed that people with psoriatic arthritis have more sleep problems than those who dont. One reason is the pain and discomfort of the condition, but other links are being explored. Trouble falling asleep or staying asleep at night worsens fatigue the next day. Whats more, poor sleeping routines tend to take hold over time. The longer they go on, the harder they are to break. Try incorporating these practices for quality sleep into your bedtime routine:

  • Set a consistent sleep-wake schedule

  • Keep your bedroom cool, dark, and quiet

  • Limit caffeine late in the day

  • Once in bed, avoid reading, working, using a laptop or phone, or watching TV

Keep a sleep journal to share with your doctor to help you pinpoint your problem areas and identify sleep disorders such as sleep apnea or restless legs syndrome . Symptoms of sleep apnea include: stopping breathing while youre asleep, waking up suddenly gasping for breath, and loud snoring. The main symptom of RLS is an uncontrollable urge to move your legs due to unpleasant sensations.

If sleep problems persist, ask your doctor if a sleep study could help. Some studies can now take place at home.

In 2018, a reported that 67.7% of people with PsA experienced poor sleep quality, compared with 14.6% of people in a control group.

Depression and fatigue go hand in hand. People with psoriatic arthritis are at a greater risk of depression according to the National Psoriasis Foundation. The reasons for depression may go beyond the obvious discomfort and uncertainty of the disease. Scientists are now exploring a link between the inflammatory response and depression. Treating the immune response may have a direct, biomechanical effect on depression. Communicating how you are feeling can put you on the path to more energy.

Diet And Exercise To Boost Energy

Your diet can play a significant role in your energy levels, so its important to focus on nutrient-packed whole foods. Whole grains, vegetables, and fruits are better choices than empty carbohydrates and packaged snacks, while fish and lean poultry are better than eating red meat every day. Depending on your overall health, you might consider enlisting a registered dietitian to help.

When you have both pain andlow energy levels, exercise may be the last thing you want to do. However, regular physical activity can improve your overall symptoms by:

  • increasing energy-inducing endorphins
  • keeping your joints conditioned
  • improving quality of sleep at night

If you havent exercised in a while because of your condition, its important that you talk to your doctor first. Walking and swimming are easier on the joints while still providing resistance and aerobic effects.

Psoriasis is a chronic skin disease of immune systems, occurring in approximately 2% of the population , which has a significant impact on the patients life, especially in the case of progression of severe lesions . Until recently, psoriasis as a disease was assessed only on the basis of the surface and clinical image of affected skin. However, recent studies have shown the relation of psoriasis severity with physical, emotional and social function . Psychosocial aspects of disease so far have been the subject of many studies , while the fatigue as a symptom that accompanies the disease was less often the subject of researchers interest. Chronic fatigue associated with psoriasis may affect up to 50% of patients , and up to 25% of all patients indicated the severity of fatigue as severe . No association with the activity of disease evaluated with the Psoriasis Area Severity Index was also described by Tobin et al. however, fatigue rated as severe was twice as frequent in patients treated systemically than with phototherapy .

Minor pain and stiffness of mild PsA can be alleviated with non-steroidal anti-inflammatory drugs . In addition, injections of corticosteroids may be used.5

For moderate to severe disease, treatments that target joint disease in PsA frequently can reduce symptoms and prevent disease progression. Recommended treatments include disease-modifying anti-rheumatic drugs .

The first step for treatments is usually DMARDs such as methotrexate, leflunomide, or sulfasalazine. Other treatments include medicines that target tumor necrosis factor , a chemical that produces a wide range of inflammation in PsA.

Examples of TNF blockers include etanercept , adalimumab , infliximab , golimumab , and certolizumab pegol . Other DMARDs that have proven effective in clinical trials include ustekinumab , and secukinumab .1

The FDA has also recently approved Inflectra , a biosimilar to infliximab, for the treatment of psoriatic arthritis.7Physical and occupational therapy can be critical treatment approaches to both protect the involved joints and maintain function.5

Maximize Your Psoriatic Arthritis Treatment

Few of us want to take more medication than we absolutely need to. However, if you are living with psoriatic arthritis, staying on a treatment plan that keeps your symptoms in remission is an important strategy. In at least 11 studies, those who were on biologics for psoriasis or psoriatic arthritis treatment reported reduced fatigue.

How Can I Manage Fatigue

The good news is that there are ways you can deal with fatigue:

  • Talk to your doctor. Make sure you let your doctor know if your fatigue does not improve, or is made worse by your arthritis medicines. Sometimes small changes in treatment can make major differences in how you feel.
  • Exercise.Exercise is one of the best things you can do to combat fatigue. Exercise can help strengthen muscles and increase your fitness. Within a few months, you should feel an increase in strength and energy and be able to do more without getting as tired. The key to a successful exercise program is to begin gradually, listen to your body and build slowly. You may also find it helpful to get advice from a physiotherapist or exercise physiologist.

  • Limit caffeinated drinks to five or less per day, and avoid these types of drinks after dinner.

Enlist their help and understanding. Educate those close to you about fatigue being a part of your arthritis, and explain to them the ups and downs you experience.

    Most people understand what you mean when you say youre tired weve all been there. When youre tired, you feel as if your body is dragging. You might crave a bit more sugar and caffeine than usual for energy. Perhaps you can sneak in a short nap and wake up feeling a little perkier. You cant wait to crawl under the covers and get a good nights rest so you can wake up and feel better the next day.

    But what if you wake up after seven to eight hours of sleep and feel as though you can barely get out of bed? What if you feel like you cant mount the energy to do much of anything and the persistent, nagging exhaustion keeps preventing you from participating in your usual activities?

    This is fatigue, which is much different than feeling tired.

    Fatigue is a major symptom for roughly 30 to 40 percent of patients with psoriatic arthritis, says rheumatologist Alexis Ogdie, MD, Director of the Penn Psoriatic Arthritis Clinic in Philadelphia and Associate Professor of Medicine at the Hospital of the University of Pennsylvania.

    Fatigue is not quenched by sleep alone, says Dr. Ogdie. You have to break the cycle at multiple points and address different aspects of fatigue for it to lift. Sleep is part of it, but you also have to address inflammation, pain, itchy or irritated skin, exercise, depression and anxiety, stress, other contributing health issues, and more.
